2010 Human Rights Breakfast ANNUAL AWARD NOMINATION FORM (Must be returned by November 6, 2010) Since 1973 the Tampa/Hillsborough Human Rights Awards have recognized those individuals or entities who have served the community by championing human rights for all residents. There are several award categories although any individual or entity may only be recognized in each category once in their lifetime. The Award Committee reserves the right to select multiple or no winners in any category each year. Awards Breakfast will be held December 10, 2010 at the Doubletree Westshore Hotel, 7:30am. We are seeking nominations from community members. Please check the box to indicate the award for which you are sending a nomination: 2010 HUMAN RIGHTS HERO (NEW) Must meet the following criteria: 1. A Hillsborough County resident who has demonstrated a long term commitment and lasting impact in the area of human rights. A person with a ‘body of work’ that shows an unwavering commitment and impact on the community. A person recognized as a ‘pioneer’ in their field who has repeatedly led efforts to promote human rights. 2010 HUMAN RIGHTS AWARD Must meet the following criteria: 1. Provided human service – related support to persons living in Hillsborough County beyond those services for which they have been provided compensation, i. e. , they must have volunteered these services. 2. Provided these services without regard to race, sex, religion, age, national origin, marital status, disability, sexual orientation, familial status or any other basis that may be used to discriminate against people. 3. Supported human rights issues for a period of time that extends as a minimum, beyond the year in which he/she is being nominated for. Additional information may be attached. 2010 HANK WARREN MEMORIAL AWARD Must meet the following criteria: 1. Served residents of Hillsborough County as an elected or appointed governmental official for a minimum of one (1) year. May be a current or former office holder. 2. Championed the cause of human rights by supporting legislation, policies, practices and activities that further the cause of fair opportunities for all citizens in employment, housing, public accommodations, healthcare, educational opportunities and other areas that affect human existence. 3. Demonstrated initiative and innovative dedication toward the establishment and support of programs, activities and practices that support the cause of human rights and that are considered pace-setting accomplishments. YOUTH HUMAN RIGHTS LEADERSHIP AWARD (NEW) Must meet the following criteria: 1. Be a Hillsborough County resident under the age of 25. 2. Demonstrated leadership in the area of human rights in a school or community setting that includes gaining the support of multiple partners or supporters and encompasses more than one event or initiative. High School Essay Contest
Theme:
“Human Rights Pioneers: Who’s your Hero?”
Since 1973 the Tampa/Hillsborough Human Rights Council has recognized local human rights champions. The Essay Contest is presented to challenge students to consider the importance of human rights and how they apply in our community. Awards are given to the top three writers as selected by an independent panel of local educators. Any student in grades 9-12 in Hillsborough County is eligible.
Awards will be presented at the 37th Annual Human Rights Awards Breakfast on Friday, December 10, 2010 @ 7:30 AM at the Double Tree Westshore Hotel in Tampa. Awards include:
First Place…………. $300.00 Savings Bond, Trophy, 2 tickets to Awards Breakfast
Second Place…………$200.00 Savings Bond, Trophy, 2 tickets to Awards Breakfast
Third Place…………$150.00 Savings Bond, Trophy, 2 tickets to Awards Breakfast
Honorable Mentions will receive Certificates. The decision of the judges is final.
How to enter:
1. Write your essay in no more than 600 words. Print legibly or type. Essay should not identify the student or school.
2. Prepare essay cover sheet with name, age, grade, school, email, phone and home address as well as name and phone of contact person from your school.
3. Submit essay and cover sheet by deadline of November 6, 2010.
